5th Grade Yearbooks and Fun Day T-Shirts

It is bittersweet to be sending out the form for the 5th Grade Yearbook and Fun Day T-Shirt order!  The yearbook captured many memories of the 5th graders’ time at North School that they will be able to take with them as they continue on.  

The t-shirts will be worn by all 5th grade students at their year-end celebration on Fun Day and will display all of the 5th grade students’ names on the back of the shirt.  Please indicate how the student’s first name should appear in the “First name on t-shirt” space below.

T-shirts and yearbooks are for 5th grade students only.  Additional yearbooks and t-shirts may not be purchased.

Delivery of the yearbooks and the date of the Fun Day will be announced later.

Orders can be placed using this form with cash or check (made payable to North School PTA), submitted to school in an envelope marked “5th Grade Yearbook” OR through our new online processing system below.  Please note, orders placed online incur a $0.85 convenience fee.

Orders are due by Friday, April 20th.
